After School Information
The Boys & Girls Club is providing transportation from 4 schools in Milford: JFK, Pumpkin Delight, Matthewson and Meadowside There are 50 spots available on each bus on a first come first serve basis. You will not be added to the bus list unless your payment is made and all required forms are handed in. A waiting list will follow.THE FOLLOWING RESTRICTIONS APPLY:
1. All transportation must be for every school day to the Boys & Girls Club of Milford. There will be no split service, such as, 3 days at one location and 2 days at another.
2. It will take one week to process new transportation requests. This allows all parties including the Clubhouse, the Bus Company and Milford Public Schools to coordinate.
3. Completion of this form does not guarantee transportation. Seating is limited and is on a first come, first serve basis.
4. Online Membership Registration, Payment and all forms must be completed prior to riding the bus to the Boys & Girls Club of Milford.
5. At any time, the Boys & Girls Club of Milford reserves the right to revoke transportation privileges based on behavior. Your membership and transportation fee is nonrefundable.
The After School Program for 2016-2017 will be held at Simon Lake Unit located at 59 Devonshire Road (Old Simon Lake School, rear entrance) for grades Kindergarden (must be 6 years old) through fifth grade and at our Teen Center located at 14 Benham Ave. The cost of the After School Membership is $100.00 for the school year. The After School Program runs from the first to the last day of school with the Milford School District. The Simon Lake Unit is open Monday thru Friday 3:00pm to 6:00pm. The Teen Center is open 2:00pm to 8:00pm. If you are signing up for transportation service, the fee for this is $250 for the year and must be paid at the time of registration. All forms and payment must be in by 6pm August 24th in order for your child to ride the bus the first week of school. Children will be seperated into groups based on their grade level.
SUMMER CAMP 2017 IS NOW FULL. WAITING LIST OPEN
The Summer Camp Program 2017 is a licenced summer camp for ages 6 to 15 years old and is located at 59 Devonshire Road in Milford. There are 180 spots available. The camp runs from July 3rd thru August 11th, Monday thru Friday 7:30am to 6:00pm. The cost of our summer program is $650.00 for the full 6 weeks. Should you choose to send your child for single weeks the cost is $125.00 a week. Your spot is not secure until registration, forms and payment are complete. Required forms for camp are Membership Disclaimer, Household Income, and recent physical form. A birth certificate is required for all newly enrolling 6 year olds.
Early Bird Registration Fees
Before May 1st-Full Summer Pricing- $575 for 6 weeks. Weekly Pricing-$100 per week. May 1st and beyond the normal prices apply. 25% must be paid at the time of registration to secure your spot. 25% payment due May 1st, June 1st and before July 1st. All fees are non refundable and space is limited.
I further acknowledge that the failure by me or my child to follow the rules may result in my child's suspension or expulsion from the Club. I agree that the Club will not be responsible for any accident to my child on the Club's premises or while engaged in any of its activities. I agree that photos of my child may be used in public relations materials, I give my consent for any photographs in which my child may appear to be used in any way the Club may choose to use them for that purpose. I UNDERSTAND that my child's membership is not complete until all documents and payments are made at the Clubhouse. Please note, the Summer Camp 2017 fees listed are the early bird fees. Registrations May 1st forward are $650.00 for Summer Camp and $125.00 a week if selected seperately.
